Rehabilitation Worker I - HART Hub - Smiths Falls

As a Rehabilitation Worker I, you will support clients living in residential settings as they work
toward their recovery and life goals. You will play an active role in helping individuals build the
skills needed for greater independence, including activities of daily living, healthy lifestyle
practices, social relationship development, and effective use of leisure time.
This position is based within the HART Hub, a 66-bed, two-site initiative providing a full
continuum of addiction and mental health services across Lanark, Leeds, and Grenville counties.
This role is specifically located within our 8-bed residential addiction treatment program, which
is co-located with 12 supportive housing spaces. The HART Hub is designed to break down
traditional service silos, integrating withdrawal management, intensive treatment, supportive
treatment, and long-term housing to provide client-centered, continuous care.
As a Rehabilitation Worker I within the HART Hub, you will contribute to a therapeutic, safe,
and supportive environment. You’ll be responsible for helping clients maintain a clean and
comfortable living space, attending to their well-being and safety, and supporting their journey
through structured programming and daily living routines. This is a front-line opportunity to
make a meaningful difference in the lives of individuals recovering from substance use and
mental health challenges.
Qualifications & Requirements
Qualifications:
Graduation from a recognized post-secondary institution with a minimum 1-year degree/diploma in a health-related field such as Social Service Worker, Mental Health and Addiction Worker, Community and Justice Services
Two years’ experience working in a health-related field is preferred.
Current CPR and First Aid is required.
Valid Ontario Driver’s license and access to a vehicle required.
Valid vehicle insurance with liability of $2,000,000 minimum.
Demonstrated crisis intervention skills/experience.
Ability to work within the Mental Health Act, Occupational Health & Safety Act, Ministry of Health and Long-Term Care guidelines, Psychosocial Rehabilitation Practitioner’s
