How to Refer Yourself
Self-referral is welcome and encouraged. Call our central number to speak with an intake coordinator who will help connect you to the right services.
Referring Someone Else
Family members, friends, physicians, and community organizations can make referrals on behalf of individuals (with consent). Contact us to discuss appropriate services and the referral process.

All LLGAMH services are accessed through our Centralized Intake. Our "one door – one number" approach makes it easy to find the right support without knowing in advance which service you need.

Assessment
Our intake team will complete a brief assessment to understand your situation and identify the most appropriate services.
Service Connection
You will be connected with the right program and staff. Services are typically covered by OHIP.
